Application to be a part of an upcoming Thrive Mission Trip

ACKNOWLEDGMENT, ASSUMPTION, AND RELEASE


I, the undersigned, wish to participate in a short-term mission project conducted under the auspices of Thrive Church (“Thrive”).  By signing this form, I acknowledge (1) that traveling to and in a foreign country involves hazards not customarily encountered when traveling in America. (2) Medical facilities in other countries may be substandard and that should a medical emergency develop during my trip, it is unlikely that I will receive medical care in that country equivalent to that available in America. (3) Working conditions in foreign countries are often inferior to conditions in America. (4) Thrive does not carry insurance to insure against any of the risks I may encounter in another country.


Despite the foregoing, it is my desire to participate in foreign mission work, and I knowingly assume the risks that are involved and release Thrive, its employees and agents, from any liability for injury, damage, or harm which may occur to my person or property while traveling in connection with this project or otherwise participating in this project.


I affirm that I am eighteen (18) years of age or older, or the parent/guardian of the participant if under eighteen years of age, and that this Acknowledgment, Assumption, and Release is binding on me and my executor, administrators, and heirs.  I give Thrive and its representative (s) with me on any such trip authority to request and authorize medical and/or hospital treatment for my benefit in the event of any injury or sickness sustained by me while on such ministry activity, including, without limitation, while traveling to and from any foreign country.  I agree to pay for all such treatment and to reimburse Thrive for all costs and expenses incurred by it with respect to such treatment.


As required by Privacy Regulations, Thrive Church may not use or disclose your protected health information without your authorization.  


I hereby authorize Thrive Church and any of its employees to use or disclose the health information provided to Thrive to medical professionals, medical institutions, or Thrive short-term mission team members in a foreign country or in the US and to act on my behalf in case of a medical emergency.


Health Information authorized to be disclosed includes medications, health conditions, allergies, and all other health information provided to Thrive Church.


Effective dates for this authorization are from the date the team departs and will expire at the end of the trip upon the teams return.

Thrive Church Mission Trip Conduct and Dress Code     


Each mission team member is highly visible in the communities where we serve. We must always be aware that our actions can either help or harm the work that the Lord is doing through our teams. There are community norms and safety measures that we ask be observed during the entire duration of your trip.


Please read and follow all the guidelines of the Thrive conduct and dress code policy. Your submission of this online form is a commitment to adhere to the policies stated here.


  1. Seek to exhibit the attitude of a humble servant.
  2. Be flexible.
  3. Seek to keep an attitude that promotes unity, cooperation and respect between the team and those in leadership. (Team leaders, local mission partners, etc.)
  4. Sleep only in the room to which you have been assigned.
  5. Travel, work and eat as part of the team, never wandering off on your own.
  6. Be willing to accomplish any task assigned to you or the group.
  7. Be willing to dress by the standards which are appropriate for the culture, seeing to never be a distraction or bring offense by what you wear. Adhere to the dress code. No exceptions. Team leaders, as well as our mission partners, may deem attire inappropriate for a situation and require a change of wardrobe before participation is permitted.
  8. Abstain from any conduct as requested by leaders or which you know could be offensive to other or contrary to a clear testimony for Jesus Christ. This means refraining from any use of tobacco, alcoholic beverages, offensive language or abusive drugs. Keep your conduct with the opposite sex at the highest moral standard of respect, modesty and morality as well as anything else that would negatively reflect or impact your witness or bring a bad light upon the partner ministry or Thrive Church.
  9. Show respect and understand that the doctrinal beliefs and practices of Thrive will be held as the standard while serving on location.
  10. Abstain from arguments, particularly those regarding politics, religion or traditions.
  11. Attempt to attend all team meetings prior to departure and during the mission trip.
  12. Do not seek out romantic relationships with another team member, team leader, national leader, translator or anyone you might meet on this trip.
  13. Refrain from giving gifts such as money to the nationals. Although we know your intent is good, these actions can result in problems and even security concerns. Please consult with your team leader regarding any needs you observe that might be served through the provision of funds before taking any action.
  14. As a short term missionary, remember your responsibility to refrain from any other behavior or activity that would hinder your ministry or the ministry of your team during the term of your service.

Safety

Leave all expensive jewelry and electronic items at home! Items you bring you do at your own risk. Thrive is not responsible for any broken, misplaced, or stolen items. Unless with the group for an activity, stay inside the hotel or mission house rooms after sunset. NEVER go anywhere alone or without the permission of the team leader. 


Dress

All clothing should be modest and free from any questionable designs. The team leader or mission partner leadership may require a change of attire before participation is permitted if clothing is deemed inappropriate. You will want good walking shoes or work boots/mud boots depending on the work you will be doing and the conditions at the mission location. Open toe shoes such as flip flops or sandals are not permitted on the mission field but may be worn around the mission house/hotel area after hours.


  • Women & Girls
  • Skirts & dresses must reach the top of the knee or longer, longer is preferred! If wearing shorts, they MUST reach the top of the knee. No short-shorts, cheerleading shorts, yoga shorts or stretch pants. No low necklines. No abdomens or midriffs showing at any time. No cropped shirts or blouses, halter tops or spaghetti straps. If bathing suits are used on the trip, only one piece suits are permitted and an appropriate cover up for use out of the water is required. Dresses are suggested if the team will attend church.
  • Men & Boys
  • Pants or shorts to the knee or longer are required. Shirts are required at all times. Tank tops and cut out sleeves are not permitted. If bathing suits are used on the trip, shorts to the knee/board shorts are required. Khakis or similar pants with a collared shirt are suggested if the team will attend church.


Modesty of dress is a requirement of being part of this mission team! Parents and adults taking responsibility for a minor are expected to supervise the packing, attire and conduct of their child for the duration of the mission trip.


Release of liability and agreement to all conduct and dress code policies

I agree not to hold any person, group, church or organization, including Thrive, its Board of Directors, or the leadership team, liable for any injury, damage, or loss connected with this mission trip. I also agree to follow the directions of the team leader and all conduct and dress code policies required as a missionary traveling with Thrive Church.
